If you do really want to know without waiting though later on in the game you’ll find a tape called “Happy Birthday” which shows you what happens to clancy. The tape is optional but the location can’t really be missed as it’s near a key item. as for the “L” on the paper, it stands for everybodies favourite psychopath “Lucas”

One of the tooltips in 21 clearly states that Lucas waa having a laugh at Clancy and Hoffman because they didn’t realize they were infected already and thus their fingers and such would grow back. So Clancy could have easily have lost fingers etc. and had them grow back by the time Happy Birthday happened.
