You will only be able to choose your difficulty when starting up a new game (or New Game+). Choose wisely as you will have to stick with it for the rest of the game. Can be changed once on Standard If you choose Standard, you will have a chance to lower the difficulty to Casual from the Game Over screen.

Awesome game. I finished the first play through on Standard difficulty, which seemed about right. Not too hard, not too easy.
